How to Use the Student Reward and Discipline Template
To download our free template file to your computer, just follow the link at the bottom of this page.
The next step will be to enter the names of all the students you want to track in the first column of the template. When you need to change the example given or add a new name just click the cell you want to edit or add and write something new.
Now, go to the next column and write in the action the student continues to do that is disruptive to the classroom or your set of rules.
The next section is for entering the consequences of those actions. This way, you can build a 3 strike system and keep track of their offenses.
Likewise, you will want to praise students for their good behavior and you can note that in column E. If you would like to leave yourself notes, you can do so in the last column of the template.
